I had this problem, too. What I found was that I had the .htaccess file in
/usr/local/nagios/share, which covered the initial login, but when I went
to any of the cgi pages, it would report 'logged in as ?'. The fix was to
copy the .htaccess file from /usr/local/nagios/share to
/usr/local/nagios/sbin. The reason that it is happening is that you are
authenticated to the HTML files' directory, but not to the CGI files'
directory.
