I think the reason is that you didn't read the documentation and
didn't configure Nagios and/or your web server properly. There can be
other reasons for this error message, which I can't really rule out
based on the "extensive" description on what you have done and what
your configurations are. 

You might want to have a look at
http://nagios.sourceforge.net/docs/1_0/toc.html, especially the
chapters about installing Nagios and Configuring Nagios.
