We had a somewhat similar problem with Nagios 1.0 (or Nagios 1.1, I
forget which now) where a notification would be generated on the
transition from OK to WARNING but we'd never get a notification on the
WARNING to CRITICAL transisition. (We noticed it on file system
monitors, most of our monitors go directly from OK to CRITICAL.)
I "solved" the problem by upgrading to Nagios 1.2. I never looked to
see what the problem was, solving the problem by running portupgrade
was easy enough. :)
While not directly on point, I hope this helps a bit.
