Try one of the following ones
a)
snpp -n -m \"Host: $HOSTNAME$ Host: $HOSTALIAS$ Address: $HOSTADDRESS$
State: $HOSTSTATE$ Info: $OUTPUT$ Date: $DATETIME$\"
$CONTACTPAGER$@test1
b)
snpp -d -s SendpageServer -n -m "Host:$HOSTALIAS$ IP:$HOSTADDRESS$
State:$HOSTSTATE$ :$DATETIME$" $CONTACTPAGER$@test2

a) was my old working config (But I had now problems with Nagios 1.2)
b) is currently working in my test installation.
