What operating system and version?

What HTTP server software and version?

What version of Nagios?

What is the *exact* error?  Which HTTP code (500, 404, etc)?

Is there anything in the HTTP server logs pertaining to this problem?

Do other CGI programs work?

If it's Apache, do you have "ScriptAlias" configured as described in the
installation documentation at
http://nagios.sourceforge.net/docs/1_0/installweb.html?
