The thing that bit me here was that I had failed to put my .htaccess file

Not sure if this is your problem or not.. but you might want to confirm
that
your .htaccess file is in both the CGI directory and the root of the HTML
directory, and that it is readable by your web server user.
While trying to solve my own problem, I threw together a quick little CGI
to
check that Apache was passing auth information properly... drop a copy of
ftp://ftp.conundrum.com/pub/software/env.cgi in your Nagios cgi-bin directory
and then try to run it. Make sure Apache is setting HTTP_USER correctly.
