Did you verify the notification tag? "notify-phone-by-email" doesn't
match my: "notify-by-email", "notify-by-pager", notify-by-skytel"
commands.

You might dump out the entire templates:

define contact{...}
define command{command_name notify-by-email...}

Just my thoughts.
