Host Notifications Disappearing

Hi Folks:
I'm running a distributed nagios 1.2 configuration, and I had a couple of
host notifications that never showed up. I was doing escalation testing
today, and everything worked. I regenerated configurations with the
correct IP's so that the device would show up again. The logs detect
that it came back up but no notifications were received and the
notification history cgi shows nothing either.
The template I'm using has that this device type should notify from both a
remote monitoring host, and a parent aggregate host. On both servers it
did not notify. It is configured to notify on d,u,r.
The following are a sample of the logs
Jun 24 13:52:23 host nagios: Warning: Host check command 'check-host-alive' for host 'testhost' timed out after 10 seconds
Jun 24 13:54:22 host nagios: HOST NOTIFICATION: test;testhost;DOWN;host-notify-by-email;(No Information Returned From Host Check)
Jun 24 14:07:24 host nagios: Warning: Host check command 'check-host-alive' for host 'testhost' timed out after 10 seconds
Jun 24 14:22:23 host nagios: Warning: Host check command 'check-host-alive' for host 'testhost' timed out after 10 seconds
Jun 24 14:24:25 host nagios: HOST ALERT: testhost;UP;HARD;2;(Host assumed to be up)
No host notification. The server would have been restarted to get the
updated host IP, so when it started back up, it assumed it was up...
Any ideas. Servers are running version 1.2.
