Reading status for Nagios

I want to connect our Asterisk PBX to Nagios to allow the PBX to decide
what way to route calls based on the status of the route in Nagios. For
example, we have two internet providers, and two voip providers, one on
each internet connection. If route A has packet loss or is down, that
is, Nagios flags as WARN or FAIL, I wold like Asterisk to route the
calls to the other, and if both are down, thrugh PSTN. How can I have
the Asterisk server query Nagios for a specific service?
