Ok, I had this same problem some time ago... First, you must to retire the default_user_name at the cgi.cfg archive. You must define a valid contact at the minimal.cfg (or another archive that you have your contacts). This contact must be in htpasswd archive... Try to do it... I realize that Make sure that you have the correct htaccess information for the _sbin_ directory, not (just) the share directory. Depending on how you configured it it's either in httpd.conf or an .htaccess file in ~nagios/sbin.
