Are you using state retention? Per the documentation,
notifications_enabled is an entity for which the value in the retention
file overrides the value in the configuration file. See the Retention
Notes and further documentation at
http://nagios.sourceforge.net/docs/1_0/xodtemplate.html. I strongly
suspect that is your problem.
